Required Forms for all Grants
Below please find the five required forms for most grants managed by our agency.
W-9 Form (PDF)
Completion of the IRS W-9 form is required for each agreement; including exempt entities. We are required to withhold twenty percent (20%) of any payment to an individual or company for which we do not have either a social security or a Federal Tax I.D. Number. If you believe payments to you by the WMD are exempt from the IRS reporting requirements, please provide an explanation of the exemption, with reference to the appropriate IRS regulations providing for such exemptions.
Signature Authorization Form (PDF)
Identifies the persons in your organization who have authority to sign grant agreements, amendments, and requests for reimbursements.
Debarment Certification Form (PDF)
This certifies that your organization will not use funding from this grant agreement to do business with any person or organization debarred from doing business with the U.S. government.
2 CFR Part 200 Subpart F Audit Certification Form/Federal Funding Accountability and Transparency Act (FFATA) Form (Word)
The Subrecipeint shall also comply with the Federal Funding Accountability and Transparency Act (FFATA) and related OMB Guidance consistent with Public Law 109-282 as amended by section 6202(a) of Public Law 110-252 (see 31 U.S.C. 6101 note) and complete and return to the Department an Audit Certification/FFATA Form, provided by the Department. This form is required to be completed once per calendar year, per Subrecipient, and not per agreement. The Department’s Contracts Office will request the Subrecipient submit an updated form at the beginning of each calendar year in which the Subrecipient has an active agreement.

Energy Facility Site Evaluation Council grants: Funds to conduct radiological emergency preparedness programs relating to the Columbia Generating Station nuclear power plant.
U.S Department of Energy grants: Funds to conduct and maintain emergency preparedness programs relating to the Hanford Site.
