Washington Youth Academy Educational Records Request
When a student completes the residential phase of the National Guard Youth Challenge Program, the official grade report and other educational records (assessment scores, discipline record, immunization history, community service summary) are not automatically sent from the Washington Youth Academy to the high school the student last attended, since not all students return to their former schools.
When a cadet enrolls at the next high school or educational institution, a request to transfer records (“records request”) is usually generated by the registrar or other official of the receiving school or educational institution, and is mailed or faxed to the registrar of the Washington Youth Academy. This request, sent on the receiving school’s letterhead and signed by a school official, is the preferred method for transferring educational records from the Washington Youth Academy to the cadet’s next educational institution.
However, there are some educational institutions that require the parent, legal guardian, or adult student to personally initiate a records transfer. Additionally, there may be times that parents, legal guardians, or adult students need records sent to themselves and/or other agencies (for example, proof of high grade average for insurance, employment references, or documents for military recruiters). In these cases, records requests initiated by a parent, legal guardian, or an adult student may be sent on a Washington Youth Academy Academic Records Request Form or on a separate sheet of paper. Please note that records requests initiated by a parent, legal guardian or adult student require the following information:
- Student’s full name (no nicknames)
- Birth date
- Session attended (e.g., 2010-01)
- Current daytime telephone number
- Items to be released
- Complete name and address of where to send the copy(s) or who is authorized to pick-up
- Signature and date
If student is under 18 years of age, the parent/guardian must sign. If the student is over 18 years of age, the student must sign.
All requests for student educational records must be in writing and must be signed by an authorized individual. The signed requests may be mailed, faxed, or delivered to:
